Case 10: Physical Exam

Vital Signs: Temperature 100.5 °F,  Blood Pressure 96/58 mmHg, Heart Rate 106 beats/min, Respiratory Rate 18 breaths/min, Oxygen Saturation 99% on room air
General Appearance: Awake, alert, mild distress related to abdominal pain
HEENT: Normal conjunctiva, no scleral icterus, dry mucous membranes, clear oropharynx
Lymph: No lymphadenopathy
Cardiovascular: No JVD, regular rhythm, no murmurs, rubs, or gallopsstethoscope
Pulmonary: Clear to auscultation bilaterally, no wheezes, no rhonchi
Abdominal: Soft, distended, mild diffuse tenderness, tympanic to percussion, hypoactive bowel sounds
Extremities: Warm and well perfused, no pitting edema
Neurologic: Alert and oriented, no focal neurological deficits

Psych: Calm, pleasant
Skin: Warm, dry, no rashes
