Case 2: Differential Diagnosis

Build your Differential Diagnosis:

What’s at the top 3 of your differential diagnosis?
Alcoholic Hepatitis
Less likely given absence of significant alcohol history and lack of jaundice. Try again!
Acute Viral Hepatitis
Good choice! This is a good option given the history of IV drug use
Drug-Induced Liver Injury
Good choice! This is a good option given the history of the patient attempting suicide. She denies herbal or dietary supplements but over the counter medications such as acetaminophen can be a culprit.
Autoimmune Hepatitis
Less likely given the lack of comorbid autoimmune disease or family history.
Infectious Gastroenteritis
This is possible given severe nausea and vomiting but is less likely to have systemic symptoms. Try again!
Wilson Disease
Acute liver failure secondary to Wilson disease should be ruled out in young patients with underlying psychiatric disorders. The labs will help evaluate further.
