Vital Signs: Temperature 98.1℉, Blood Pressure 100/65 mmHg, Heart Rate 110 beats/min (Sitting: BP 92/50 mmHg, HR 132 beats/min), Respiratory Rate 18 breaths/min, Body Mass Index 20 kg/m2
General Appearance: Chronically ill appearing
HEENT: PERRLA, EOMI, b/l scleral icterus, +conjuctival pallor
Cardiovascular: Tachycardic, normal S1/S2, no rubs/murmurs/gallopsstethoscope
Pulmonary: Clear to auscultation bilaterally, no wheezing, rales, or rhonchi
Abdominal: Soft, nontender, distended, +fluid wave, normoactive bowel sounds
Extremities: Cold extremities, 2+ pitting edema b/l lower extremities
Neurologic: Alert and oriented to person, place, and time, no asterixis
